Medication
All medications, prescription or over the counter:
1. Must have written authorization from physician and parent (signed med sheet)
2. Must be in the original pharmacy container with the correct label and dosage.
In June, a new med sheet will be mailed home for completion and returned to the Health Office every September, or if playing a fall sport, before the start of the first practice in August.
